About The Position

St. Croix Hospice is seeking a compassionate Registered Nurse to serve as the first point of contact for patients and families facing a terminal diagnosis. The Admissions Nurse travels to patients wherever they reside, conducts comprehensive assessments, and gathers clinical information to support timely, appropriate hospice care. The nurse provides education and guidance to families as they begin the hospice journey, develops individualized care plans, identifies immediate and ongoing needs, and establishes trust through clear, empathetic, and professional support. The position offers meaningful patient connections and autonomy in practice.

Requirements

• Current Registered Nurse (RN) license in the state(s) of practice; multistate license accepted where applicable.
• Ability to pass a background check, drug screening, and consumer report.
• Valid driver’s license, insured reliable vehicle, and acceptable driving record.
• 1+ years of hospice or home care experience and 2+ years of overall nursing experience preferred.

Responsibilities

• Perform nursing care in accordance with the Nurse Practice Act, professional standards, and hospice guidelines.
• Complete initial and ongoing patient and family assessments.
• Develop, implement, and update individualized Plans of Care in collaboration with the care team.
• Provide direct patient care, including medication administration, symptom and pain management, and activities of daily living support.
• Educate and support patients and families.
• Maintain accurate documentation and HIPAA compliance.
• Travel to patients wherever they reside to conduct admissions and assessments.
