About The Position

Amedisys is seeking a full-time Admissions Registered Nurse to provide home health care in Sussex County. The nurse performs patient start-of-care assessments, develops and implements plans of care, and provides skilled nursing services in accordance with the plan of care and agency policies. The role also includes patient education, coordination with care team members and community resources, and referrals when appropriate. The posting lists an annual pay range of $85,000–$95,000 and a $10,000 bonus.

Requirements

• Current RN license specific to the state(s) of assigned work.
• One year of clinical experience as an RN; candidates with less than one year must be approved by the VP of Clinical.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Perform comprehensive patient start-of-care assessments and evaluations, and develop and implement plans of care.
• Complete OASIS assessments accurately and on time.
• Provide skilled nursing care in accordance with the plan of care and agency policies.
• Communicate with staff, physicians, family members, and community resources to support continuity of patient care.
• Initiate appropriate referrals when indicated.
• Promote and maintain patient health through teaching and appropriate rehabilitative measures.
• Help patients learn appropriate self-care techniques.
• Perform other duties as assigned.
