About The Position

Triniti Home Health & Hospice is hiring a full-time, benefit-eligible RN Admissions Nurse based at its Metro Branch in Edina, Minnesota. The nurse provides a smooth transition for patients by building relationships and coordinating the admission process. The position evaluates patient referrals, manages home health admissions, completes OASIS, collaborates with case managers, maintains relationships with referral sources, documents referral records and reports, and anticipates and orders equipment needed for a patient’s episode. The role suits someone who can manage a fast-paced environment, multitask, and provide strong customer service.

Requirements

• Must be a licensed RN in the State of Minnesota.
• At least 2 years of experience in skilled nursing, the admission process, discharge planning, or management.
• Strong knowledge of local, state, and federal guidelines related to home health care.
• Ability to comprehend clinical and technical data.
• Strong leadership, written communication, and verbal communication skills.

Responsibilities

• Evaluate patient referrals, manage home health care admissions, and complete OASIS.
• Coordinate with the Case Manager to ensure a smooth patient admission.
• Maintain positive relationships with current referral sources.
• Document and maintain paperwork, referral records, and reports.
• Anticipate and order equipment needed for a patient’s episode.
• Perform other duties as assigned.
