About The Position

Sunnyside Nursing and Post-Acute Care is a skilled nursing and post-acute care facility in Torrance, California, providing sub-acute nursing care, rehabilitation, skilled nursing, and long-term care. The Admissions Nurse (LVN) provides hands-on assessment and nursing care during the admissions process, evaluates patient needs, and initiates care plans for admissions and readmissions. The role includes communicating assessment findings and physician orders, maintaining timely and accurate documentation, and contributing to an interdisciplinary team. The nurse may assist with medication pass when needed. The schedule is full-time, Tuesday through Saturday, 3:00 p.m. to 11:30 p.m.

Requirements

• Current and active California LVN license.
• Authorization to work in the United States.
• Prior Admissions Nurse experience at a skilled nursing facility.
• Willingness to perform medication pass when needed.
• Comfort with PCC.
• Ability to read, write, speak, and understand English.

Responsibilities

• Provide hands-on assessment and care to patients during the admissions process.
• Communicate assessment findings effectively to the physician.
• Deliver knowledgeable and competent quality nursing care to patients.
• Evaluate patient assessment needs and initiate care plans upon admission and readmission.
• Accurately receive, document, and transcribe physician orders.
• Communicate orders promptly to ensure timely receipt of medications and implementation of orders.
• Chart accurately and in a timely manner.
• Provide excellent customer service and maintain a positive attitude toward patients.
• Assist with medication pass when needed.
• Maintain dependable, regular attendance.
• Use concentration, reasoning skills, and good judgment.
• Communicate and function productively as part of an interdisciplinary team.
