About The Position

The Clare is recruiting a hospitality-focused Admissions Nurse to clinically admit residents to Skilled Nursing. The role supports the Admissions & Outreach Manager with all aspects of the admissions process, from initial inquiry through move-in, and works with the LCS Clinical Consultant to ensure consistency with LCS standards. The position requires compliance with federal, state, and local regulations governing admission practices. The schedule is 11:00 a.m. to 7:00 p.m., Tuesday through Saturday. The salary range is $80,000–$95,000. Employees must provide proof of COVID-19 vaccination or receive an approved exemption as a condition of employment.

Requirements

Registered nursing license in the State of Illinois.
Minimum of 3–5 years of experience in admissions, intake, or case management in a senior living, long-term care, or healthcare setting.
Experience evaluating clinical information and making placement decisions.
Proficiency with Microsoft Office Suite products.

Responsibilities

Clinically admit new residents to Skilled Nursing.
Ensure the necessary equipment is in place to meet residents’ needs.
Support the Admissions & Outreach Manager in managing the admissions process from initial inquiry through move-in.
Complete onsite or virtual pre-admission assessments at hospitals and other care settings as directed.
Collaborate with the LCS Clinical Consultant to ensure consistency in LCS standards.
Ensure compliance with federal, state, and local regulations governing admission practices.
