Requirements

  • High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
  • Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Responsibilities

  • Manage the full admissions and discharge process for a skilled nursing facility, ensuring accurate documentation and smooth, timely transitions for residents and families.
  • Act as the main point of contact for patient inquiries, referrals, and facility tours—providing quick responses and a friendly, helpful experience in a fast-paced healthcare environment.
  • Review and organize patient referral and clinical information to support appropriate admissions decisions and ensure the facility can meet each resident’s care needs.
  • Track and maintain daily census, referral activity, and admissions data with strong attention to detail, keeping records accurate and up to date.
  • Support healthcare operations by assisting with scheduling, staffing coordination, and workflow organization to keep the facility running efficiently and delivering high-quality care.
