ABOUT THE ROLE Position: Transitional Aged Youth Case Manager Type of role: full time, 37.5hrs/ week Location: hybrid (Oakville; traveling across Halton Region may be required) Posting is for an existing vacancy.

Requirements

  • University degree/ diploma in Social Work and/ or Health-related field plus Addiction/ Concurrent Disorders studies certification, or equivalent combination of experience and education.
  • Registered Social Service Worker is an asset.
  • Current CPR and First Aid certification is required.
  • ASIST training or equivalent is required.
  • CPI training is an asset.
  • CACC certification is an asset.
  • At least 2 years working experience with youth and young adults in the area of addictions or related field.
  • Experience in: Community development and networking; Record keeping and file management; Basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work; Applied knowledge of addictions treatments and explanatory theories, mental illness, and community resources; Managing clients with complex needs in varying stages of change.

  • Experienced sense and awareness of ethical boundaries and clinical reporting obligations
  • Ability to successfully deliver culturally responsive services.
  • Cultural knowledge of key populations in Halton Region that will be served to address disparities in specific services.
  • Ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, and races.
  • Knowledge of the history of discrimination in America, particularly as it relates to race and racism, and how this history has led to disparities experienced by marginalized communities such as BIPOC, people with disabilities, LGBTQ+ communities, and others.
  • Knowledge of French language an asset
  • Proficiency with computer programs and electronic database(s)
  • Must undergo a police record check

Responsibilities

  • Deliver clinical services to Transitional Aged Youth (TAY), ages 16 – 24, by providing clinical assessments and developing and implementing clinically sound treatment plans.
  • Provide intensive case management support in an outreach model of service.
  • Work directly/ cooperatively with the ADAPT TAY Coordinator/ Youth Addictions Manager and team as well as with TAY Project Partners and a wide range of service providers, to increase service integration for transitional aged youth.
  • Provides specialized assessments to Transitional Aged Youth, or to collateral clients associated with the youth, through the use of diagnostic tools and structured interviews.
  • In consultation with the Client, develops an individual Treatment Plan for the Client.
  • Provides individual, couples, family and/or group counselling to address the Clients’ needs.
  • Conducts group sessions as required. DBT skills training is an asset.
  • Refers Clients to other appropriate support services and agencies according to the Clients’ needs, with specific attention to supporting referrals between youth and adult mental health and addictions services.
  • Participates in multi-agency TAY consultations and all related TAY committees.
  • Conducts appropriate follow up with respect to the Client’s treatment plan.
  • Provides consultative services to community agencies, professionals or individuals as required.
  • Maintains accurate and complete client records according to guidelines established by the MOHLTC or the funding agency.
  • Assists in the promotion of ADAPT and its programs to other agencies and the general public.
  • Participates in meetings as required.
  • Provides input into the operational and strategic planning related to ADAPT programs as required.
  • Assists in maintaining a high level of open and effective communications within the organization.
  • Provides other services according to the conditions of program funding.
  • Participates in committees as required.
  • Participates in community education and public awareness initiatives as required, with specific focus on items related to TAY clients and services.
  • Participates in performance evaluations as required.
  • Participates in clinical supervision sessions as required.
  • Participates in professional development activities as required.
