ABOUT THE ROLE Position: 2 x PHAST Navigator Type of role: full time, 37.5hrs/ week Schedule: rotating shifts; days (8:30-4:30), evenings (12:00/1:00-8:00/9:00-tbd); regular Saturdays (10:00-6:00) Location: hybrid (hospital; traveling across Halton Region may be required; home office) Posting is for existing vacancies.

Requirements

  • University degree/ diploma in Social Work/ Social Service and/ or Health related field plus Addiction Studies certification
  • Current CPR and First Aid certification is required.
  • ASIST training or equivalent is required.
  • CPI training or equivalent is required.
  • At least 5 years of relevant experience
  • Experience in: Individual, family, and group counselling
  • Experience in: Community development and networking
  • Experience in: Record keeping and file management
  • Advanced life skills counselling, supportive counselling, crisis intervention, advocacy, and group work
  • Managing clients with complex needs in varying stages of change
  • Strong communication and active listening is required
  • Ability to effectively present complex clinical information in writing and verbally
  • Ability to read and understand documentation such as operating instructions, procedures manuals and complex clinical reports.
  • Broad knowledge of addictions treatments and explanatory theories, mental illness, and community resources.
  • Ability to: function independently, maintain effective working relationships with others, work effectively under pressure or deadlines and work outside regular office hours as required, use good judgement to make sound decisions
  • Interpersonal skills that require an attention to detail and rapport building with clients
  • Experienced sense and awareness of ethical boundaries and clinical reporting obligations
  • Ability to successfully deliver culturally responsive services
  • Cultural knowledge of key populations in Halton Region that will be served to address disparities in specific services
  • Ability to communicate and effectively interact with people across cultures, ranges of ability, genders, ethnicities, and races
  • Knowledge of the history of discrimination in America, particularly as it relates to race and racism, and how this history has led to disparities experienced by marginalized communities such as BIPOC, people with disabilities, LGBTQ+ communities, and others
  • Active time management and scheduling
  • Proficiency with computer programs and electronic database(s)

Nice To Haves

  • Professional registration with the Ontario College of Social Workers and Social Service Workers an asset
  • Knowledge of French language an asset

Responsibilities

  • Provide group and individual counselling and facilitate active transfer and navigation of clients to subsequent needed services across the region.
  • Addictions and mental health assessment, delivery of acute stabilizing interventions, evaluation of interventions and treatment
  • Facilitate development and implementation of clients personalized wellness plans
  • 1:1 Instrumental support based on needs
  • Group counselling development, delivery and evaluation
  • Crisis support
  • Family engagement, support and education
  • Maintaining proper health records at a high standard of quality
  • Community engagement
  • Interprofessional, inter-agency engagement, relationship building and collaboration
  • Identify next stage services and facilitating navigation and warm transfer to these services
  • Complete statistical information in accordance with Ministry and Program requirements
  • Participate in continuous quality improvement cycles
