About The Position

Pathways to Life, Inc. is seeking an ACTT Team Leader in Greenville, NC. This role involves leading a team of 6-8 members providing community-based services to adults with serious mental illness (SMI) and substance use disorders. The position requires a licensed clinician (LCSW, LCMHC, or LMFT) with at least one year of experience working with these populations. Responsibilities include conducting assessments, developing treatment plans, delivering therapy, providing crisis response, and supervising team members. The role involves significant field work in various community settings, including homes, shelters, and hospitals, as well as office/admin time for documentation and supervision. The team includes psychiatry, nursing, therapists, and peer support. This is a full-time position with a Monday-Friday schedule, 8:00 AM to 5:00 PM, and includes participation in a 24/7 on-call rotation.

Requirements

North Carolina License: LCSW, LCMHC, or LMFT
Minimum 1 year of experience working with adults (paid or volunteer)
Direct work experience with SMI, substance use, or crisis populations
Valid Driver's license
Ability to travel daily
Ability to lift up to 25 lbs

Responsibilities

Complete biopsychosocial and substance use assessments.
Build treatment plans with the ACTT team.
Deliver therapy using Motivational Interviewing, Cognitive Behavioral Therapy, and relapse prevention techniques.
Provide field-based care to clients in homes, shelters, ERs, and community locations.
Run 2-4 person-centered meetings per week.
Support client stabilization, including medication adherence, coping skills, and anger regulation.
Respond to behavioral health crises in the community and de-escalate situations.
Guide 6-8 team members on clinical decisions.
Coordinate with psychiatry, nurses, employment specialists, and peer support.
Review documentation to meet Medicaid and ACTT standards.
