About The Position

Pathways to Life is seeking a compassionate and motivated ACTT Qualified Professional (QP) to support adults living with severe and persistent mental illness throughout the Greenville and Pitt County area. Since 2006, Pathways to Life has provided community-based mental health and substance use services focused on restoring wellness, independence, and hope. Our mission is to help individuals and families achieve wellness through compassionate, evidence-based care delivered by qualified professionals dedicated to positive change. This position offers the opportunity to make a meaningful impact while working alongside a collaborative multidisciplinary team committed to recovery-focused care. Assertive Community Treatment Team (ACTT) is an evidence-based, intensive community treatment model designed for adults with severe and persistent mental illness who may struggle with traditional outpatient services and are at increased risk for hospitalization, homelessness, crisis episodes, substance use, or incarceration. Often described as a “hospital without walls,” ACTT delivers comprehensive behavioral health services directly in the community, including homes, shelters, hospitals, workplaces, and other community settings. The goal of ACTT is to help individuals: Remain safely in the community Reduce psychiatric hospitalizations Improve independence and quality of life Strengthen daily living and coping skills Achieve recovery and personal goals Services are individualized, flexible, and centered on each member’s needs and strengths. The ACTT Qualified Professional provides intensive, person-centered behavioral health interventions, crisis support, rehabilitation services, and care coordination in community and field-based settings. The QP works closely with psychiatrists, nurses, therapists, peer support staff, and substance use professionals to support members with symptom management, independent living skills, medication adherence, housing stability, employment goals, and community integration. Services are delivered in accordance with NC DHHS, NC Medicaid, CARF, HIPAA, and ACTT fidelity standards.

Requirements

  • Bachelor’s degree in Psychology, Social Work, Counseling, Human Services, or related field
  • Must meet NC DHHS Qualified Professional (QP) requirements
  • Minimum two (2) years of experience working with individuals with mental health or co-occurring disorders
  • Valid driver’s license, reliable transportation, and current automobile insurance

Nice To Haves

  • ACTT, CST, IIH, or community-based behavioral health experience
  • Experience working with severe and persistent mental illness
  • Preferred credentials: LCSW / LCSWA LCMHC / LCMHCA LCAS / LCAS-A Or equivalent behavioral health credentials

Responsibilities

  • Provide intensive community-based behavioral health services to ACTT members.
  • Conduct assessments, interventions, rehabilitation services, and crisis response in home, office, hospital, and community settings.
  • Support members with: Symptom management, Medication adherence, Independent living skills, Housing and employment goals, Social and community integration.
  • Develop and monitor Person-Centered Plans (PCPs).
  • Utilize motivational interviewing and assertive engagement techniques.
  • Participate in rotating on-call and crisis response responsibilities.
  • Coordinate services with providers, hospitals, families, and community resources.
  • Complete accurate and timely EHR documentation.
  • Maintain compliance with Medicaid, DHHS, CARF, and agency standards.
