About The Position

The Activities Support Provider assists with the day-to-day operations of Clarity Care’s Day Services Program. The role supports individuals with varying abilities in pursuing independent, fulfilling lives in their communities, with responsibilities spanning goal setting, independence, community involvement, consumer rights, personal care, daily living activities, and regulatory compliance. The ideal candidate is compassionate, people-oriented, comfortable working as part of a team, and willing to provide hands-on support with daily activities. The provider helps members take part in community and recreational activities, meet personal goals, make decisions, and enjoy engaging activities tailored to their interests. The role also includes preparing and serving meals according to members’ dietary needs and preferences.

Requirements

High school diploma or equivalent and/or related work or life experience.
Must be at least 18 years of age.
Valid Wisconsin driver’s license and ability to meet agency driving standards.
Proof of auto insurance.
Must pass a periodic Wisconsin Caregiver Background Check, including Department of Justice criminal record checks and/or checks for previous states of residence.
No experience or certifications are needed.
