This is a full-time Clinical Coordinator opportunity for a Registered Nurse (RN) managing with a CRRT program with Inpatient Dialysis. The role is primarily day shift. The Clinical Coordinator uses advanced nursing processes in decision-making, patient assessment, data analysis, development and implementation of methods and systems to improve the overall quality and efficiency of patient care for the designated department and population served. They provide direct and indirect patient care as needed, oversee the delivery of patient care, and provide day-to-day management of programs to coordinate patient care as designated by the department. The role coordinates the delivery of safe, high-quality patient care, identifies areas for study, and participates in and/or supports research. The Clinical Coordinator utilizes advanced nursing processes in clinical decision-making, patient assessment, and care coordination to improve quality, efficiency, and patient outcomes for the assigned population. They provide direct and indirect patient care as needed and oversee the delivery of care within the designated department. The role coordinates interdisciplinary care plans, ensuring alignment with patient needs, clinical guidelines, and organizational goals. They perform patient assessments, facilitate referrals, and ensure appropriate placement and transitions across the continuum of care. The Clinical Coordinator collaborates with physicians, nursing staff, social work, and ancillary services to ensure coordinated and effective patient care. They identify and address barriers to care and implement strategies to improve patient outcomes, including length of stay and resource utilization. The role develops, analyzes, and evaluates clinical and operational data to identify trends and support quality and performance improvement initiatives. They ensure compliance with regulatory and accreditation requirements, including The Joint Commission (TJC) standards. The Clinical Coordinator provides education to patients, families, and staff to support understanding of care plans, treatment options, and post-discharge needs. They serve as a resource, mentor, and educator to staff and team members. The role facilitates communication among patients, families, and care teams to ensure continuity and quality of care. They participate in interdisciplinary team meetings and contribute to care planning and progression discussions. The Clinical Coordinator supports departmental and system-wide performance improvement and research initiatives. They maintain accurate and timely documentation in accordance with organizational and regulatory standards and advocate for patients and families throughout the care experience.
