About The Position
The YVLifeSet specialist is responsible for providing services to address the needs of young people 17-21 who are transitioning out of care and need individualized skill development and support to transition successfully into adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement.
Requirements
- Requires a bachelor’s degree in the human services field
- Requires a valid driver’s license and vehicle insurance.
- Requires at least one (1) year of supervised related experience in counseling and/or case management.
- State Police Clearances
- FBI Clearances
- Child Abuse Clearances
- Mandated Reporter-Recognizing and Reporting Child Abuse training certificate and/or statement
Nice To Haves
- master’s degree preferred
- Two (2) years of experience is preferred
Responsibilities
- Gathers and understands current and past psychosocial histories.
- Provides clinical direction that meets the needs of the young adult.
- Provides support to young adults in times of crisis.
- Monitors and records young adults’ progress.
- Assesses risk factors related to safety and well-being of the young adult.
- Delivers services outlined in the service plan.
- Complies with Pressley’s standards and state licensing requirements.
- Exemplifies the values of the program service model.
- Maintains a caseload census within the program model expectations.
- Meets with young adults at least weekly to provide skill coaching identified in the service plan.
- Maintains flexibility and creativity in working a non-traditional schedule to accommodate the needs of young adults and their identified supports.
- Collaborates with young adults and their natural support system, internal and external stakeholders, program team members, supervisor and organizational support staff.
- Provides advocacy for young adults to outside agencies.
- Receives supervision.
- Receives supervision from and actively communicates with supervisor on an ongoing basis throughout the week in person, by phone, email, and/or written message regarding program matters.
- Participates in group supervision on a weekly basis to present and review service plans and discuss client needs.
- Participates in consultation with Licensed Program Expert on a weekly basis to present and review service plans and discuss client needs.
- Works with team members to develop ownership for the service plan.
- Plans and leads treatment team meetings for the purpose of creating service plans.
- Writes and utilizes specific measurable goals, objectives, and time frames for each young adult.
- Uses a strength focused, solution-based approach to provide comprehensive information for a service plan on a monthly basis.
- Develops crisis plan for each young adult.
- Evaluates and reforms goals and objectives as necessary.
- Completes all paperwork in a timely and professional manner.
- Independently seeks out resources and learning opportunities to foster ongoing professional growth.
