About The Position

This position provides case management services to families eligible for the YMCA Caregiver Supports Program, specifically supporting licensed and unlicensed caregivers of youth in foster care. Case management involves coordinating or providing services to ensure children and caregivers can access necessary medical, educational, social, clinical, and other services for the child's physical, psychological, and developmental health. The role also includes providing leadership and support to the Case Management team, ensuring quality case management for all clients in the Caregiver Supports program. This role is part of the Y Social Impact Center, the social services division of the YMCA of Greater Seattle, serving youth, young adults, and families across King County, Pierce County, and other WA counties. The Y Social Impact Center focuses on strengthening young people's potential by reducing risk factors, navigating systems, establishing stability during crises, and building competencies and social capital. Their programs encompass homelessness prevention, housing, behavioral health, crisis response, foster care licensing, violence prevention, and employment. The organization values relatable lived experience, especially for those involved in foster care, criminal justice, behavioral health, and homelessness systems.

Requirements

Bachelor's degree in psychology, social work or related social services field or equivalent experience, Masters preferred. Other applicable education, training, and experience, which provide the knowledge, abilities, and skills necessary to perform effectively in the position will be considered.
Experience working with teens or young adults in employment, foster care, group care, housing, education, juvenile justice, anti-poverty, vocational support, mental health, chemical dependency and/or homelessness fields.
Ability to teach effective independence skills for a variety of learning styles and to plan supportive activities.
Strong youth engagement and relationship-building skills.
Strong verbal and written communication skills.
Basic computer skills and working knowledge of Microsoft Office.
Ability to transport self and client(s) in a safe and effective manner, and to meet clients at community locations; proof of adequate vehicle insurance coverage is required, or ability to travel to areas without public transportation.
Current state approved First Aid and CPR certification.
Current approved HIV blood borne pathogens training.
Must be 21+ to be an authorized driver.
Must have possessed a driver's license for at least 3 years.
Must pass a drug screen that will test for impairing substances (including marijuana).
Must meet the YGS Driving Record Standards: Within the last three (3) years: No more than two (2) vehicle related suspension(s) and/or reinstatement(s). No more than two (2) moving violations and/or accidents. Within the last five (5) years: No Major Violations (DUI, DWI, OUI, OWI, BUI, Administrative Per Se, or Refusing to Test; Driving with an open container of alcohol; Reckless Driving; Hit and Run – property damage only; Racing; Driving with a suspended/revoked license/insurance; Speeding over 20 mph; Fleeing and/or eluding police; Any other felony driving conviction(s)).
Driving records that result in Permanent disqualification from driving duties: More than 3 DUI, DWI, OUI, OWI, BUI, Administrative Per Se, or Refusing to Test for entirety of driving history; More than 3 Driving with a suspended/revoked license; Hit and Run resulting in bodily injury or death; Manslaughter/felony death by motor vehicle.
Not currently able to enroll California Driver's License holders as Authorized Drivers.
The YMCA of Greater Seattle will conduct individualized assessments of applicant criminal histories.

Nice To Haves

Masters preferred.
Prefer knowledge of and previous experience with, diverse populations (language, culture, race, physical ability, sexual orientation, etc.). Ability to speak any language in addition to English may be helpful.
A Master's degree in social services or a closely related field; OR A Bachelor's degree in social services or closely related field; OR Five (5) years full-time of documented experience working with children or youth in one of or a combination of the following settings: i. State licensed in-home or facility-based daycare, or professional nanny; ii. School setting or early learning center; iii. Mental Health Hospital; iv. Juvenile Rehabilitation facility, detention center, law enforcement, or corrections; v. A child placing agency, residential care program, or as a licensed foster parent; vi. Conducting supervised visits; or vii. Working as staff directly with children or families in a community service organization.
Staff with a bachelor’s degree must consult with a person with a master's degree in social services or closely related field for one (1) hour for every twenty (20) hours the Support management employee works.

Responsibilities

Provide case management and care coordination services as outlined in the Caregiver Supports Contract, Statements of Work, Handbooks, and Policy/Procedures, to clients enrolled in the Caregiver Supports Program, including licensed and unlicensed caregivers of youth in foster care, therapeutic foster care, intensive in-home stabilization services, and Outpatient Mental Health services.
Maintain a caseload size in accordance with contractual requirements and guidelines.
Ensure clients receive all required screenings, assessments, services, interventions, and opportunities needed to fully meet their physical, psychological, social, emotional, relational, developmental, educational, independent-living, housing, and permanency needs in a timely and effective manner.
Develops individualized, strengths-based, youth-driven service plans.
Understand contract/grant terms and conditions to ensure program outcome requirements are met and services rendered to young adults appropriately.
Researches best practices in the field; creates and maintains a specialized resource library.
Maintains clear, professional, timely participant contact notes, service plans, progress updates, outcomes, and other documentation in designated databases.
Teaches life skills, job readiness, or other subject matter individually and/or in groups, often experientially.
Teams effectively and collaboratively with YMCA staff and/or community partners to plan and facilitate activities, including visits to related resources in the community.
In coordination with the Program Director, and in collaboration with the child & family team, ensure clients' transitions are well coordinated and facilitated, including placements & discharges while ensuring that all required intake and discharge/exit documentation is completed in full and on-time.
Provide leadership and guidance in the placement referral process, and ensure that case transfers (of CMs, Intervention Specialists, and Child & Family Therapists) are smooth and as least disruptive as possible to the client.
Builds and maintains collaborative relationships with funding liaisons, other service providers, businesses, and/or educational institutions as related to specialized services.
Supports participant success in key domains such as safe and stable housing, permanent connections to supportive adults, education and employment, health and well-being, and life skills.
Ensure clients' parent/child/sibling visitation needs and requirements are met. Ensure natural supports and life-long connections are developed and stabilized.
Ensure each client's file/record is fully in compliance with WAC, DSHS/DLR, DSHS/CA, DSHS/DBHR, KC-BHO, COA Accreditation, and YMCA requirements. Ensure all clients' information is kept confidential and only released in accordance with HIPAA, DSHS, and YMCA policy and procedure.
Conduct Intake Assessment and create individual service plans for non-BRS clients.
Participate in clinical consults for clients as necessary in order to be able to provide clinical advocacy for clients. In conjunction with health professionals, assess clients' mental health needs, and determine appropriate level of care and support.
May assist with program coordination or other activities related to job focus.
Participates in YMCA annual fundraising campaign to engage community members in volunteerism and philanthropic support for the Y mission.

All employees and volunteers are responsible for adhering to abuse prevention policies, maintaining appropriate boundaries, completing required training, reporting suspicious or inappropriate behavior, and complying with mandated reporting requirements.
Responsible for ensuring staff are properly screened, trained, supervised and held accountable to the organization’s abuse risk management standards.
