This position is responsible for the full client care cycle, including assessing, planning, implementing, monitoring, and evaluating actions required to meet clients’ health and human services needs. The role involves coordinating and providing safe, timely, effective, efficient, equitable, and client-centered care under minimal supervision. A significant portion of the role may involve prevention services, including education, information dissemination, participation in Positive Behavioral Interventions and Supports (PBIS) efforts, collaboration with community partners, screening for needs, and encouraging positive activities. The Case Manager must also complete all required agency, accrediting body, and funder training.
