About The Position

The Fatherhood Case Manager provides individual coaching and personalized support to fathers. Through the Fatherhood Project, coaches offer guidance on goal setting, parenting support, resource coordination, referrals, and case management. They are also trained to facilitate the Nurturing Fathers Program.

Requirements

Associate’s degree in behavioral science or related field, or equivalent work experience as allowed/required by regulatory bodies.
Driver License and valid auto insurance.
Ability to learn case management principles, healthcare management, and reimbursement requirements.
Excellent organizational and time management skills.
Effective verbal and written communication skills.
Ability to relate to and interact with a wide variety of people.
Advanced level problem-solving skills and ability to multi-task while prioritizing.
Ability to utilize technology including electronic service documentation systems.
Basic computer skills with ability to navigate Microsoft applications including Word, Outlook, Excel, Teams, and PowerPoint.

Responsibilities

Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered under minimal supervision.
Maintain caseload and utilization by regularly contacting clients to monitor and assess progress.
Handle case assignments, draft service plans, review case progress, and determine case closure for review and/or in consultation with a supervisor.
Deliver ongoing services to clients, which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral/linkage to community resources.
Assist clients in achieving wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.).
Develop and/or implement individualized client treatment or service plans and ensure compliance with applicable standards and agency policies.
Document all clinical activities accurately and timely to meet specific program, funder, and credentialing entity criteria.
Participate in treatment teams and assist in making recommendations for client status changes.
Provide client education and facilitate group curriculum in accordance with program requirements.
Adhere to professional standards as outlined by protocols, rules, and regulations.
Provide group and individual educational services on Mental, Emotional, and Behavioral (MEB) health, Social-Emotional Learning (SEL), coping skills, and related topics to students, families, educators, and community members (up to 30% of annual work time for school-based/community programs).
Share information and resources regarding MEB health issues through school, family, and community events.
Participate in Positive Behavioral Interventions and Supports (PBIS) efforts and collaborate with community coalitions to support prevention-focused policies and practices.
Collaborate with schools, community partners, and other agencies to coordinate and support prevention initiatives and services.
Screen for student, family, and community needs and connect individuals with appropriate services and resources.
Encourage participation in positive activities that strengthen protective factors, reduce risk-taking behaviors, and promote healthy attachment to families, schools, peers, and communities.
Complete agency, accrediting body, and funder required training.
Perform other duties as assigned.
