About The Position

Mission Prep Healthcare, part of AMFM Healthcare, is seeking compassionate and dedicated Community Coordinators to support adolescents (ages 12–17) at their premier Residential Treatment Facilities. This role is crucial in redefining adolescent mental health care, helping teens and their families heal, grow, and move forward with hope. The Community Coordinator is expected to cooperate with management and clinical staff, possessing the knowledge, skills, and experience to supervise client services and provide a safe, comfortable, and caring environment. The program emphasizes tight-knit connections between staff and clients, fostering trust, safety, and inclusivity for growth and learning. Mission Prep's approach treats the whole family, blending evidence-based therapies with innovative interventions in a nurturing, home-like setting.

Requirements

  • High School Diploma or GED
  • Must pass ADP Selective Screening Services and other Federal/State background checks.
  • Valid Driver’s License with ability to be insured by the auto insurance policy (requires 4 continuous years of driving experience, 21 years of age or older, less than 3 moving violations in the last 3 years, and no more than 1 major violation in the last 3 years).
  • Ability to work on a computer, walk/stand for long periods, lift up to 20 lbs., and navigate stairs.
  • Strong communication skills with families, clients, and staff.
  • Ability to remain professional, empathetic, and patient under pressure.
  • Proficiency in Microsoft Office, Adobe Acrobat, and Google Suite.

Responsibilities

  • Coordinates and transports clients safely to all daily activities, ensuring timely arrival and participant safety.
  • Utilizes intervention and communication skills in working with clients, their families, and significant others daily and in crisis.
  • Rigorously adheres to all Joint Commission policies and procedures, including Waived Testing, Infection Prevention and Control, and Emergency Management and Environment of Care procedures.
  • Collaborates with the program nurse and clinical team regarding medication management, monitors and reports client behavior with medication side effects, and documents medications according to organizational policy.
  • Maintains the integrity of the residences, ensuring optimal cleanliness and hygiene.
  • Immerses oneself in the daily lives of clients to foster a connected and harmonious environment.
  • Facilitates recovery and clinically indicated activities and groups, documenting into the electronic health record (BestNotes).
  • Conducts observation rounds to ensure client safety and documents client behavior and participation in treatment.
  • Collaborates in a multidisciplinary environment and participates in individualized, person-centered care and treatment plans, adhering to CA DSS regulations and Mission Prep Healthcare principles.
  • Conducts orientation to new residents, including reviewing safety drills and emergency procedures.
