About The Position

The Youth and Family Resource Coordinator will support JFS clients and their caregivers by connecting them to information and resources that address their individual needs. With the goal of enhancing mental health and wellbeing among youth (6-18) and young adults (18-25), the Youth and Family Resource Coordinator will foster community partnerships, make appropriate referrals, and educate clients about the resources available to them.

Requirements

  • Bachelor's Degree in related field or equivalent combination of education and relevant experience may be considered.
  • Understanding of client advocacy and mental health systems, either from educational or personal background.
  • Minimum of 2 years of professional experience working with children, adolescents, young adults, and/or families in healthcare, behavioral health, education, social services, nonprofit, or community-based settings.
  • Excellent time management and interpersonal communication skills.
  • Experience providing assessment services, case management, and/or community outreach.
  • Demonstrated ability to engage clients with empathy, professionalism, and cultural humility.
  • Familiarity with common mental health challenges faced by children, teens, young adults, and/or families.
  • Experience navigating healthcare, mental health, and/or educational systems
  • Strong understanding of community resources, service systems, and referral processes.
  • Adheres to legal and ethical standards of care, including scope of practice, client privacy and record-keeping, HIPAA, and cultural humility.

Responsibilities

  • Uses a compassionate, strengths-based lens to build trusted relationships with youth, young adults, and caregivers. Identifies clients’ needs, existing resources, and barriers to care. Partners with clients to develop service plans with measurable short- and long-term goals.
  • Improves access to critical services as a subject matter expert for young adult clients and the parents of youth clients seeking support across mental health, medical, educational, developmental, and community-based systems. Provides guidance regarding mental health services, psychological evaluations, speech and occupational therapy, specialized healthcare providers, parent education opportunities, benefits programs, and social service support. Educate clients and caregivers on provider options, referral processes, insurance considerations, costs, waitlists, and service expectations.
  • Helps clients and caregivers effectively navigate healthcare, behavioral health, educational, and community support systems. Acts as a point of coordination among clients, JFS clinical staff, schools, community providers, and other stakeholders, as appropriate. Facilitates communication and information sharing to reduce service gaps and improve continuity of care. Proactively maintains ongoing engagement with clients to monitor progress, reassess needs, and promote service utilization.
  • Partners closely with JFS clinical team and other Shalom Austin departments staff to coordinate care and ensure client needs are addressed holistically. Participates in staff meetings, trainings, supervision, organizational initiatives, and offers coverage when needed.
  • Maintains accurate, timely, and confidential records of client interactions, service plans, referrals, follow-up activities, and outcomes in alignment with established systems and processes. Monitors clients’ needs over time and contributes data-driven insights to support grant reporting and quality improvement efforts. Utilizes tools that assess service delivery and capture goal progress.
  • Develop, cultivate, and maintain strong working relationships with healthcare, mental health, educational, governmental, and nonprofit organizations throughout the Greater Austin area. Expand and maintain an up-to-date network of community resources and referral partners to support clients. Represent JFS at community meetings, conferences, outreach events, and professional networking opportunities.
