About The Position
The Arc Fond du Lac is seeking a compassionate and organized Youth Community Enrichment Case Manager to support youth participants and their families through our Youth Community Enrichment Program (YCEP). This role is ideal for someone passionate about helping children with intellectual and developmental disabilities (IDD) reach their full potential.
Requirements
- Age 21 or older.
- Coursework in Advanced Early Childhood Development or at least 2 years of experience working directly with individuals with intellectual and developmental disabilities.
- Minimum 1 year of supervisory experience.
- Valid Wisconsin driver's license with a clean driving record.
- Strong communication, organization, and leadership skills.
- Proficiency with Microsoft Office.
- Ability to pass a background check and pre-employment drug screen.
Responsibilities
- Manage a caseload of youth with IDD and develop person-centered service plans.
- Coordinate participant admissions, assessments, annual reviews, and discharges.
- Work closely with families, guardians, schools, and service providers.
- Plan engaging activities, community outings, and skill-building opportunities.
- Ensure compliance with Wisconsin CLTS Waiver regulations and documentation requirements.
- Assist with staff training, onboarding, and program oversight.
- Support participant health, safety, and overall quality of care.
- Drive accessible vehicles for community-based activities as needed.
