Requirements

  • Master’s Degree in Social Work or related field.
  • LISW or LPCC licensure with supervisory designation required.
  • Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
  • Ability to use a computer; familiarity with Microsoft Office products and Electronic Health Record (EHR).
  • Excellent verbal and written communication skills
  • Strong time management and organizational skills
  • Crisis management skills required.

Responsibilities

  • Coordinates activities related to Intensive Home Based Services.
  • Helps with intake, referrals, and placement of youth.
  • Reviews and approves Individual Service Plans and Individual Service Plan updates.
  • Provides Intensive Home-Based Services to include crisis counseling and other therapeutic services needed for clients and Therapist on his/her caseload according to each client’s needs/abilities.
  • Assesses a client’s therapeutic needs/strengths and establishing a therapeutic relationship while utilizing interventions that will enhance his overall functioning.
  • Assures programs are striving to meet the Goals related to the IHBT model of treatment and also meet the fidelity needed for TVN models of treatment.
  • Requires on-call responsibilities including, but not limited to, crisis response calls for clients assigned to you and backup for crisis response calls and on-call responsibilities (including weekends and holidays) for your IHBT team.
  • Assures the use of high risk interventions (restraints) are kept to a minimum through the Implementation the Six Core Strategies to Creating Violence Free and Coercion Free Treatment Environments.
  • Completes appropriate documentation and paperwork including, but not limited to progress notes after each therapy session, Terminations, Transfers and ISPs.
  • Serves as a contact point for families and agency by engaging in correspondence and communicating information within the confines of The Village Network’s confidentiality policies and procedures.
  • Facilitates case management by designing, writing, and coordinating individual treatment plans for each client.
  • Reads/analyzes materials and reports from outside organizations, discusses with staff the progress of the client, revision of plans in order to ensure the best possible care, directing workers on how treatment plans should be implemented and monitoring compliance with the treatment plan.
  • Interviews, evaluates, and participates in the decision process regarding new candidates to The Village Network.
  • Performs other duties as required.
