Philadelphia Youth Basketball (PYB) is a mission-driven nonprofit that leverages the power of basketball to create opportunities for young people across Philadelphia. Through a “Kinder to Career” model, PYB serves youth from early childhood through age 29, integrating basketball with academic, social, and career development. PYB delivers high-quality, community-based programming that supports youth both on and off the court—building skills, confidence, and long-term pathways to success. Anchored at the Alan Horwitz “Sixth Man” Center and supported by a network of citywide program sites, PYB provides safe, inclusive spaces where young people and community members can learn, grow, and connect. Through collaborative leadership and accessible programming, PYB serves as both a catalyst and connector within the city’s broader youth development ecosystem. To advance this work and deepen support for youth and families, PYB is seeking a Youth & Family Support Coordinator. Position Overview Reporting to the Chief Mission Officer, the Youth & Family Support Coordinator serves as a critical connector between PYB youth, families, and the network of community-based resources necessary for stability and well-being. This role provides direct case management and resource navigation support, working closely with youth and caregivers to assess needs, remove barriers, and connect them to essential services such as healthcare, housing, food access, mental health supports, and other social safety net resources. The Coordinator ensures that families are not only referred, but successfully linked to and supported through services over time. In addition to direct support, the Coordinator partners with PYB program staff to reinforce a safe, inclusive, and supportive environment for all participants. This includes contributing to restorative practices, supporting youth through behavioral or social-emotional challenges, and helping integrate resource awareness into programming and family engagement efforts. The Coordinator will also support broader wellness initiatives and community partnerships as needed, ensuring PYB remains a responsive and trusted hub of support for the communities it serves. Success in this role means youth and families are able to access the resources they need, community partnerships are activated to support PYB families, and wellness programming strengthens the broader PYB ecosystem.

Requirements

  • Bachelor’s degree in social work, public health, human services, or a related field required; Master’s degree preferred.
  • Minimum 3 years of case management, social services, or resource navigation experience.
  • Strong understanding of Philadelphia’s social service landscape, including healthcare, housing, food, and legal aid networks.
  • Demonstrated ability to build trust and rapport with diverse youth and families.
  • Experience in program development, partnership cultivation, and community engagement.
  • Excellent organizational, interpersonal, and written communication skills.
  • Ability to maintain confidentiality and uphold ethical standards.
  • Bilingual or multilingual abilities are strongly preferred
  • Pennsylvania State Police Criminal Background Check (SP4 164)
  • Pennsylvania Child Abuse History Clearance Form (CY113)
  • Federal (FBI) Fingerprint Background Check (Criminal History Report), if applicable
  • Mandated Reporter Certificate
  • National Sex Offender Registry Verification
  • CPR/AED Certificate

Responsibilities

  • Manage an active caseload of PYB youth and families requiring navigation support
  • Connect youth and families to critical resources including healthcare, mental health services, housing support, food access, legal services, and employment pathways
  • Build and maintain a trusted network of community-based providers
  • Identify systemic gaps in access and advocate internally and externally for solutions
  • Ensure follow-through so that referrals result in actual service connection and utilization, not just handoffs
  • Provide direct case management support to youth and families, assessing needs and identifying appropriate community-based resources.
  • Maintain accurate documentation in PYB case tracking systems and ensure confidentiality and compliance with organizational policies.
  • Support the design and execution of PYB’s wellness initiatives.
  • Provide direct support for youth experiencing behavior challenges.
  • Collaborate with PYB’s behavioral consultant partner.
  • Facilitate and support restorative circles and mediations.
  • Partner with program teams to implement restorative approaches.
  • Reinforce PYB’s community agreements and contribute to a safe, inclusive, and affirming environment.
  • Support staff in building capacity around trauma-informed and culturally responsive practices.
  • Partner with PYB program staff to integrate wellness and resource navigation into youth programming and family engagement.
  • Serve as a liaison to community-based partners, ensuring strong communication and accountability.
  • Collaborate with the Evaluation & Learning team to track outcomes and measure impact of resource navigation and wellness initiatives
