The Young Adult Navigator will provide supportive services focused on individualized service planning, benefit entitlement support and assistance, and supportive case management for young adult individuals aged 18-25 and/or families with children headed by young adults aged 18-25. The role involves conducting intake and needs assessments, developing person-centered service plans, providing ongoing case management and advocacy, assisting with government benefits, collaborating with team members on life skills and employment efforts, maintaining records, providing crisis intervention, and making referrals to community supports. The position also requires maintaining strong relationships with tenants and stakeholders, and participating in team meetings and supervision.
