The Prince William Area Agency on Aging is seeking a dedicated, organized, and compassionate professional to join their team as a Supportive Services Specialist. This role is essential in ensuring the effectiveness of the Supportive Services Division and its programs while working to improve the quality of life for older adults, adults with disabilities and their caregivers through access to comprehensive care. The person in this role will serve as the Supportive Services Specialist responsible for providing Care Coordination and Person-Centered Options Counseling. This role ensures compliance with the Department for Aging and Rehabilitative Services (DARS), agency policies, and program guidelines while providing high-quality care for older adults, adults with disabilities and their caregivers. This role will also collaborate with internal staff and external partners to address ongoing service needs and enhance program effectiveness. The role empowers older adults, adults with disabilities, and their caregivers by connecting them with essential services and resources that promote independence, dignity, and quality of life. This involves conducting in-home assessments and reassessments, developing person-centered service plans, coordinating services with caregivers and community partners, and providing ongoing support to help individuals achieve their goals and remain safely in their homes and communities whenever possible.
