The 412 Youth Zone is located in downtown Pittsburgh. The program provides comprehensive, welcoming, and youth driven services and support to young adults transitioning out of the foster care system or who are experiencing unstable housing. The Young Adult Case Manager will develop and maintain strong, trusting relationships with young people to foster engagement and support their individual growth. This role involves conducting regular meetings with young people in their homes, community settings, or at the drop-in center to evaluate their needs, address challenges, and celebrate successes. Collaboration with community partners and service providers is key to coordinate care, maximize support, and ensure young adults have access to necessary resources. The position also requires maintaining accurate and timely documentation for effective tracking and reporting, facilitating and promoting various programs at the drop-in center, and teaching young people essential adulting skills such as budgeting, home maintenance, and decision-making.
