About The Position

The Wraparound and Special Services Director leads and supports teams across service areas to develop, implement, and support creative, strength-based plans for children and families with complex needs. The role focuses on creative solutions rather than traditional case management and is based out of any CHWC office, with day travel within New York State using an agency vehicle. The Director engages youth, caregivers, and county stakeholders; facilitates planning meetings; identifies needs; coordinates support teams; and monitors the progress and effectiveness of plans. The role also provides ongoing guidance, service coordination, advocacy, and resource connections as the needs of youth and families evolve. The Director collaborates with internal teams and external stakeholders, models and trains others in the planning process, and completes documentation and reports in a timely manner.
