Requirements

• Associate degree or higher
• 1+ years of experience in Home Health Care or Home Health
• 1+ years of experience in Electronic Medical Record (EMR), Other EMR, or Practice Partner EMR
• Valid Licensed Practical Nurse (LPN) certification/license in the state of Illinois (IL)
• Valid Certified Wound Ostomy and Continence Nurse (CWOCN) certification/license
• Valid Regular Driver License (DL) certification/license in the state of Illinois (IL)

Responsibilities

• Manage and deliver patient care plans collaboratively within a team
• Provide wound care services to patients in the coverage area
• Maintain accurate and timely charting and documentation
• Use EMR software and mobile devices for patient care management
• Drive to patient locations within the assigned coverage area
