Part Time Wound Care Nurse Practitioner or Physician Assistant - Troy, MI
About The Position
Theoria Medical is seeking a part-time Advanced Practice Provider (APP) or Physician Assistant (PA) based in Troy, MI, to serve local skilled nursing facilities. This role is part of a mission-driven care community that emphasizes relationships, consistency, and presence, allowing providers to care for patients over time and build trust. The model provides flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team. The technology includes an EHR designed to streamline workflows, voice dictation and AI Scribe for documentation, and access to UpToDate® for clinical guidance. The work schedule offers flexibility with 6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.), freedom from on-call and overnight requirements, and timely charting using a company-provided iPad with LTE. Requirements
- Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
- Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities
- Provide high-quality, patient-centered care to residents in skilled nursing facilities
- Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
- Communicate with facility nursing staff via phone or secure messaging as needed
- Maintain reliable on-site attendance for scheduled shifts
- Ensure compliance with clinical standards, facility policies, and regulatory requirements
