About The Position

Theoria Medical is seeking a part-time Advanced Practice Provider (APP) or Physician Assistant (PA) based in Troy, MI, to serve local skilled nursing facilities. This role is part of a mission-driven care community that emphasizes relationships, consistency, and presence, allowing providers to care for patients over time and build trust. Theoria Medical offers a unique blend of medical excellence and technological advancement, serving the post-acute sector. The model provides flexible scheduling and autonomy, supported by intuitive technology and a collaborative clinical team. The technology includes an EHR designed to streamline workflows, voice dictation and AI Scribe for documentation, and access to UpToDate® for clinical guidance. The work schedule offers flexibility with 6-hour facility shifts for full-time positions (rounds generally start between 7–10 a.m.), freedom from on-call and overnight requirements, and timely charting using a company-provided iPad with LTE. Requirements

  • Advanced Practice Providers must hold an active U.S. license and be board certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants).
  • Qualified specialties include but not limited to Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.

Responsibilities

  • Provide high-quality, patient-centered care to residents in skilled nursing facilities
  • Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues
  • Communicate with facility nursing staff via phone or secure messaging as needed
  • Maintain reliable on-site attendance for scheduled shifts
  • Ensure compliance with clinical standards, facility policies, and regulatory requirements
