About The Position

The Wound Care Nurse - LPN plays a critical role in providing specialized nursing care focused on the assessment, treatment, and management of patients with acute and chronic wounds. The nurse will collaborate closely with interdisciplinary healthcare teams to develop and implement individualized care plans tailored to each patient's unique needs. Requirements

  • Licensed Practical Nurse (LPN) license in good standing within the United States.
  • Completion of an accredited practical nursing program.
  • Basic Life Support (BLS) certification.
  • Demonstrated experience or clinical exposure in wound care or related nursing practice.
  • Strong knowledge of infection control standards and wound care protocols.

Nice To Haves

  • Certification in Wound Care (e.g., Wound Care Certified - WCC) or enrollment in a wound care certification program.
  • Experience working in a healthcare setting specializing in wound management, such as hospitals, long-term care, or home health.
  • Familiarity with electronic medical records (EMR) systems and documentation software.
  • Excellent communication skills for patient education and interdisciplinary collaboration.
  • Ability to work independently and as part of a multidisciplinary team.

Responsibilities

  • Perform comprehensive wound assessments including measuring, documenting, and photographing wounds to monitor progress.
  • Develop and implement individualized wound care plans in collaboration with physicians, nurse practitioners, and other healthcare professionals.
  • Administer wound treatments such as dressing changes, debridement, and application of topical medications following established protocols.
  • Educate patients and caregivers on wound care procedures, infection prevention, and lifestyle modifications to support healing.
  • Maintain accurate and detailed patient records, ensuring compliance with healthcare regulations and facility policies.
  • Monitor patients for signs of infection or complications and promptly report changes in condition to the healthcare team.
  • Participate in quality improvement initiatives and stay current with best practices in wound care through ongoing education.
