About The Position

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in promoting regulatory compliance and clinical excellence. Success requires effective teamwork, flexibility and adaptability to changing duties and pace, and a warm, encouraging disposition.

Requirements

• Graduate from an accredited school of practical nursing.
• Hold a current nursing license and state registration as an LPN or RN.
• Have a working knowledge of state regulations, policies, and procedures relevant to compliance.
• Demonstrate teamwork, flexibility, adaptability, a warm and encouraging disposition, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing is preferred.

Responsibilities

• Assist with coordinating resident admissions, including screening, assessment, and identifying care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments and ensure medications are administered according to physicians’ orders.
• Assist with coordinating care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain a working knowledge of state regulations, policies, and procedures.
