About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in ensuring regulatory compliance and clinical excellence. Success in the position requires effective teamwork, flexibility, adaptability to changing duties and pace, and a warm, encouraging disposition.

The position is part-time and hourly, with compensation of USD 32.75–USD 34.50 per hour.

Requirements

• Graduate from an accredited school of practical nursing.
• Hold a current nursing license and state registration as an LPN or RN.
• Have working knowledge of state regulations, policies, and procedures relevant to compliance.
• Demonstrate compassion for residents, eagerness to learn, and a positive disposition.

Nice To Haves

• Three years of experience in assessment and geriatric nursing is preferred.

Responsibilities

• Assist with coordinating resident admissions, including screening, assessment, and identification of care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments, ensuring medications are administered according to physicians’ orders.
• Help coordinate care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of applicable state regulations, policies, and procedures.
