About The Position

The Wellness Nurse supports residents’ health and well-being through ongoing clinical monitoring and support. The role assists community nurses, caregivers, and medication personnel in promoting regulatory compliance and clinical excellence. It requires effective teamwork, flexibility, adaptability to changes in duties and pace, and a warm, encouraging disposition. This is a part-time position.

Requirements

• Working knowledge of state regulations, policies, and procedures required for compliance.
• Graduate of an accredited school of practical nursing.
• Current nursing license and state registration as an LPN or RN.
• A smile, eagerness to learn, and compassion for residents.

Nice To Haves

• Three years of experience in assessment and geriatric nursing.

Responsibilities

• Assist in coordinating resident admissions, including screening, assessment, and identifying care needs.
• Ensure care is delivered as indicated in resident care plans.
• Administer medications and treatments, ensuring medications are given according to physicians’ orders.
• Assist with coordinating care and services with outside providers, including resident physicians.
• Document and report care provided and resident observations, including unusual or significant changes.
• Maintain working knowledge of applicable state regulations, policies, and procedures.
