The responsibility of the Wellness Nurse (WN) is to supervise delivery of care and services to residents. In achieving this goal, the WN performs procedures within the guidelines of all state and local regulations and accepted standards of care. Ongoing communication between the WN, associates, the residents and their families, professional consultants, and third-party providers is critical to the welfare of the residents.

Requirements

Active LPN or RN license required
Minimum 1 year senior living experience required
Strong clinical judgment and communication skills
Dependable, compassionate, and team-oriented
Ability to work scheduled part-time daytime hours
Must be a minimum of 18 years of age.
Licensed Practical Nurse in good standing with the state licensing agency, as required by state regulations.
CPR/AED certified.
Must have at least one (1) year of experience working as an LPN in a senior care setting.
Must have a positive Criminal Background Screening.

Nice To Haves

Previous experience with computerized resident documentation systems a plus.

Responsibilities

Administer medications and treatments per physician orders
Monitor residents and respond to changes in condition
Support caregivers and care staff during shift
Communicate with physicians, families, and providers as needed
Ensure accurate documentation and regulatory compliance
Deliver compassionate, person-centered nursing care
Help promote resident wellness and quality of life
Provide daily supervision and leadership to assigned non-licensed associates and other designated personnel in meeting the residents needs and monitor the condition of residents in AL.
Demonstrate good decision-making skills.
Make work assignments and meet with associates regularly to assist in identifying and correcting problem areas, and/or the improvement of care and services.
Provide resident care according to health care providers orders and in accordance with regulatory requirements, recognized nursing practice, established standards, and Company policy.
Accurately administer prescribed medications, supervise the continuity of resident care, respond to changes in resident condition, medications, or other circumstances requiring special care needs and perform first aid as required.
