About The Position

The Wellness Nurse provides care to residents in an assisted living community, delivering services tailored to their individual needs. The role focuses on comprehensive resident care, accurate recordkeeping, and regular monitoring. The nurse works with the wellness team and other healthcare professionals to support residents’ health, rights, preferences, and quality of life. The position is full-time, Monday through Friday from 8:00 a.m. to 4:30 p.m., with rotating weekends on call and rotating weekday call. A criminal background screening is required.

Requirements

Possess a valid and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
Communicate effectively with residents, families, and healthcare professionals, explaining medical information clearly.
Analyze situations, make sound decisions, and provide appropriate solutions in a timely manner.
Demonstrate high ethical standards, respect residents’ privacy, and maintain professionalism.
Maintain strong recordkeeping skills.
Provide care with kindness and empathy.
Complete a criminal background screening.

Responsibilities

Collaborate with a team to develop, implement, and review personalized care plans for residents.
Conduct health assessments to identify medical concerns and changes in residents’ health.
Work with other healthcare professionals to create effective care plans.
Administer prescribed medications accurately and in accordance with protocols.
Monitor medication effectiveness and promptly address side effects.
Maintain accurate records of medical information, care plans, and incidents to support regulatory compliance and consistent care.
Respond to medical emergencies, provide immediate care, and coordinate with emergency services when needed.
Communicate with residents’ families about residents’ health and any changes.
Advocate for residents’ rights and preferences.
