Nurse RN / LPN Per Diem
About The Position
The Wellness Nurse provides care to residents in an assisted living community, meeting their individual needs while maintaining accurate records and regularly checking on residents. The role works with a team and other healthcare professionals to support residents’ health, safety, and quality of life. Full- or part-time availability is offered, with hours for Sundays, Mondays, Thursdays, and Saturdays. A criminal background screening is required. Candidates must provide valid proof of full COVID-19 vaccination before starting work or commit to becoming fully vaccinated within 30 days of starting, unless granted an approved exemption.
Requirements
- Hold a valid, active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.
- Communicate effectively with residents, families, and healthcare professionals, including explaining medical information clearly.
- Analyze situations, make sound decisions, and provide appropriate solutions in a timely manner.
- Demonstrate high ethical standards, respect residents’ privacy, and maintain professionalism.
- Have strong record-keeping skills.
- Provide care with kindness and empathy.
- Complete criminal background screening.
- Provide proof of full COVID-19 vaccination before commencing work or commit to obtaining full vaccination within 30 days of commencing work, unless an approved exemption is received.
Responsibilities
- Collaborate with a team to develop, implement, and review personalized resident care plans.
- Conduct health assessments, identify medical concerns or changes, and work with healthcare professionals to develop care plans.
- Administer prescribed medications accurately and in accordance with protocols; monitor effectiveness and address side effects.
- Maintain accurate medical, care-plan, and incident records to support regulatory compliance and consistent care.
- Respond to medical emergencies, provide immediate care, and coordinate with emergency services when needed.
- Communicate with residents’ families about residents’ health and changes in condition.
- Advocate for residents’ rights and preferences.
