Requirements

  • Current, valid RN or LPN license in the state of employment
  • Knowledge of aging and disability-related care
  • Strong assessment, communication, and documentation skills
  • High attention to detail and commitment to accuracy
  • Patient, compassionate, and service-oriented mindset
  • Ability to work collaboratively and lead with integrity

Responsibilities

  • Coordinates and supports resident service plans to meet health and wellness needs while promoting a safe, compassionate, and engaging environment.
  • Helps lead and support Resident Care Aides (RCAs), ensure compliance, and serves as a trusted clinical resource for residents, families, and the care team.
  • Ensures exceptional quality of care for all residents.
  • Assesses and evaluates resident health concerns and coordinates care with providers.
  • Observes, documents, and communicates changes in resident condition clearly and accurately.
  • Provides first aid and clinical support as needed.
  • Guides, mentors, and supports Resident Care Aides (RCAs) in daily care delivery.
  • Promotes a positive, united team culture built on respect, accountability, and compassion.
  • Collaborates with team members to ensure seamless, high-quality care.
  • Serves as a liaison between residents, families, physicians, healthcare providers, and agencies.
  • Communicates professionally and empathetically with families and care partners.
  • Maintains detailed, accurate documentation and records in compliance with regulations.
  • Assists with maintaining compliance with state and federal guidelines.
  • Supports the mission, vision, and values of the community in all interactions.
