About The Position

Alto Evergreen Walk is seeking a PRN Wellness Nurse (LPN) for their Assisted Living facility in South Windsor, CT. This role is designed for individuals who prefer to work on weekends, offering flexibility and the opportunity to make a significant impact on resident well-being. The position involves supervising the care team, ensuring high-quality resident care, administering medications, and responding to changes in resident conditions. Requirements

  • 18 years or older
  • Active LPN license in good standing
  • CPR/AED certification
  • 1+ year of LPN experience in senior care
  • A clean background check
  • Commitment to a drug-free workplace
  • Must be a minimum of 18 years of age.
  • Licensed Practical Nurse in good standing with the state licensing agency, as required by state regulations.
  • CPR/AED certified.
  • Must have at least one (1) year of experience working as an LPN in a senior care setting.
  • Must have a positive Criminal Background Screening.
  • Adherence to the Company Drug Free Workplace policy.

Nice To Haves

  • Computerized documentation experience
  • Previous experience with computerized resident documentation systems a plus.

Responsibilities

  • Supervise and support the care team, monitoring resident wellbeing in Assisted Living.
  • Guide daily assignments, coach the team, and solve problems.
  • Deliver resident care in line with provider orders and nursing best practices.
  • Administer medications accurately, respond to changes in condition, and handle first aid.
  • Provide daily supervision and leadership to assigned non-licensed associates and other designated personnel.
  • Make work assignments and meet with associates regularly to assist in identifying and correcting problem areas, and/or the improvement of care and services.
  • Provide resident care according to health care providers orders and in accordance with regulatory requirements, recognized nursing practice, established standards, and Company policy.
  • Accurately administer prescribed medications, supervise the continuity of resident care, respond to changes in resident condition, medications, or other circumstances requiring special care needs and perform first aid as required.
  • Other job duties as assigned.
