About The Position

The Wellness Nurse LPN supports resident well-being at Bozeman Lodge in Bozeman, Montana. The role assists with resident admissions, assesses care needs, documents care and observations, helps ensure care follows resident care plans and applicable regulations, and responds to emergencies according to community policies. The position is full-time and day shift.

Requirements

• Current nursing license.
• Experience in community or long-term care settings.
• Geriatric assessment skills.
• Knowledge of dementia care.
• Compassion, patience, kindness, and a genuine interest in working with senior adults.

Responsibilities

• Assist in coordinating the resident admission process, including screening, assessment, and determining whether the community team can meet the prospective resident’s required level of care.
• Document and report care provided and resident observations, including unusual or significant changes in physical, mental, or behavioral conditions.
• Maintain a working knowledge of applicable state regulations, policies, and procedures to support compliance.
• Ensure care is delivered as indicated in the resident care plan.
• Respond to emergency needs as designated by community policies.
