About The Position

Physicians’ Primary Care of Southwest Florida is seeking a Wellness Nurse-LPN for its Adult Medicine Division. The nurse assists and supports physicians with annual wellness visits, creates or updates personalized prevention plans to meet patients’ health goals, and educates, counsels, and tracks patients’ progress in a wellness therapy program. The role requires maintaining strict confidentiality and adhering to HIPAA guidelines and regulations.

Requirements

• Minimum of three years of clinical experience.
• Ability to self-direct, work independently, and function well as part of a team.
• Ability to handle interactions in a professional and friendly manner.
• Excellent customer service skills.
• Prior experience creating and implementing patient care programs.
• Graduate of an accredited nursing program.
• Valid and current Florida Licensed Practical Nurse (LPN) license.

Responsibilities

• Assist and support physicians in providing annual wellness visits to patients.
• Create and/or update personalized prevention plans to meet patients’ health goals.
• Educate and counsel patients in a wellness therapy program.
• Track patients’ progress in a wellness therapy program.
• Maintain strict confidentiality and adhere to HIPAA guidelines and regulations.
