Wellness Nurse LPN
About The Position
The Wellness Nurse LPN supports resident care at Bozeman Lodge in Bozeman, Montana. The role helps coordinate resident admissions by screening and assessing prospective residents and determining whether the community team can meet their required level of care. The nurse documents and reports care provided and observations, including unusual or significant changes in residents’ physical, mental, or behavioral conditions. The position also helps ensure care follows resident care plans and responds to emergencies according to community policies. Requirements
- Current nursing license.
- Experience in community or long-term care settings.
- Geriatric assessment skills.
- Knowledge of dementia care.

Responsibilities
- Assist in coordinating the resident admission process, including screening, assessment, and evaluation of care needs to determine whether the community can meet a prospective resident’s required level of care.

- Maintain working knowledge of state regulations, policies, and procedures to support compliance.
- Ensure care is delivered as indicated in the resident care plan.
- Respond to emergency needs as designated by community policies.
