Requirements

  • High School Diploma or GED
  • Two years related community outreach and education experience, and/or training.
  • Experience working with Tribal communities.
  • Knowledgeable of Southern California Tribes.
  • Knowledge of consortium community and resources as it relates to position.
  • Ability to interpret and translate grant goals and objectives into effective services for the community.
  • Ability to work effectively with team members.
  • Ability to identify problems, use good judgment and recommend solutions.
  • Ability to establish priorities and coordinate work activities.
  • Possess excellent interpersonal communication skills.
  • Possess excellent public speaking skills.
  • A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment.
  • Current CPR certification or attainment within 6 months of hire.
  • Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test that includes marijuana.
  • Health must be adequate to perform all duties of the position.
  • Must pass a criminal background check.

Responsibilities

  • Facilitates interdepartmental meetings, ensuring information is disseminated and gathered with accuracy and cultural relevancy.
  • Clearly communicates project goals and under the direction of the COIPP Program Manager.
  • Coordinates appropriate activities/meetings to ensure goals are met.
  • Serves as a navigator to facilitate the gathering of intake and subsequent data required by funding source and determine readiness of program.
  • Works closely with coalition/advisory members, the treatment team, and outside entities to enhance the development and implementation of the community outreach and community agencies.
  • Promotes community wellness through involvement in cultural and social activities known to support individual and community resilience.
  • Maintains well-organized documentation, tracking, and reporting processes.
  • Completes project reports in a timely fashion.
  • Completes all data entry for grant funder.
  • Completes all reporting for grant reporting based on the Notice of Award.
  • Collaborates with other IHC department staff.
  • Attends department meetings, provides presentations to staff and program affiliates.
  • Participates in local community gatherings/events for outreach and recruitment.
  • Serves as Peer Support for patients and community members in COIPP Programming.
  • Complete all grant objectives based on Narrative.
  • Assists with various group workshops as planned by the Health Promotions Department.
  • Other duties as assigned.
  • Position may be re-assigned during activation of EOP.
