About The Position

Under the general supervision of the Director of Home and Community Based Services and the direct supervision of the HCBS Manager, the Navigator works as a part of the Care Management team as a support to the Care Manger I and Care Manager II on their individual Team. Navigators will assist in coordination of Case Management activities including but not limited to phone triage, scheduling visits, significant change events, HEDIS and Flu outreach, and claims review. The Navigator will document and appropriately track missed service notifications.

  • Ability to effectively communicate with the public, in person, by phone, and via email.
  • Ability to comprehend written material related to essential job functions.
  • Ability to accurately document facts related to essential job functions.
  • Ability to comprehend, recall, and apply facts related to essential job functions.
  • Ability to analyze, evaluate, and implement a reasonable course of action based on available information.
  • Ability to lift/carry 20 lbs. upstairs.
  • Ability to travel to community and consumer locations within our four-county service area (Ashtabula, Mahoning, Trumbull, and Columbiana counties).
  • Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio.
  • Ability to ambulate enough to access private homes/locations which may not be readily accessible.
  • An associate or bachelor’s degree: preferably in human services or medical field or at least 2 years’ experience in the Home and Community Based Services setting providing coordination of services.
  • Strong organizational skills with the ability to prioritize.
  • A valid driver’s license and proof of automobile insurance.
  • Must pass a criminal background check and drug screen.

Responsibilities
