About The Position

The Wellness Director / Director of Nursing at The Avenues of Fort Bend leads resident wellness and care under the guidance of the Executive Director. The role focuses on supporting residents’ independence and wellbeing through individualized, empathetic care, staff leadership, collaboration with healthcare professionals, and oversight of clinical processes and regulatory compliance. The position also includes responding to emergencies and performing other duties aligned with the community’s mission and vision. New hires must complete the screening process required by state regulations. Employment offers are contingent upon successful completion. A full job description is available upon request.

Requirements

At least 2 years of experience as a Wellness Director, Director of Nursing, or equivalent; senior care experience is preferred.
Active LVN/LPN or RN license in the state.
Strong customer service and hospitality skills.

Responsibilities

Recruit, interview, mentor, coach, and inspire the wellness team.
Perform comprehensive resident assessments, coordinate individualized care plans, and provide personalized care.
Coordinate with internal staff, third-party vendors, and medical professionals.
Lead staff meetings, manage service agreements, update physician order sheets, and ensure compliance with healthcare protocols.
Oversee medication management, complete timely audits, handle incident reporting, and conduct investigations.
Assist with resident evacuation during emergencies.
Perform other duties aligned with the community’s mission and vision.
