About The Position

Heritage Senior Living is seeking a compassionate, organized, and dependable Wellness Coordinator to help lead our resident care team and ensure residents receive consistent, high-quality, individualized care. Reporting to the Director of Nursing (DON) or designee, the Wellness Coordinator provides hands-on leadership and support to CNA/Resident Assistants while coordinating daily resident care needs, wellness activities, staffing, documentation, and communication. This is an ideal opportunity for an experienced caregiver, Resident Assistant, CNA, or care team leader who is ready to take on greater responsibility and make an impact on both resident care and team performance.

Requirements

  • High school diploma or GED required.
  • CNA certification preferred but not required.
  • Previous caregiving, Resident Assistant, CNA, senior living, assisted living, memory care, or related experience strongly preferred.
  • Previous lead, supervisor, mentor, or care-coordination experience preferred.
  • Completion and maintenance of required CBRF certifications.
  • Strong organizational and multitasking skills.
  • Ability to provide direction, coaching, and support to care team members.
  • Strong customer-service and interpersonal skills.
  • Caring and compassionate approach with genuine concern for older adults.
  • Ability to work independently while collaborating effectively with the clinical and community leadership teams.
  • Ability to appropriately recognize and communicate resident changes or concerns.
  • Ability to maintain confidential resident and employee information.
  • Competency in the safe use of floor lifts, sit-to-stand lifts, and friction-reducing devices.
  • Ability to meet applicable pre-employment, training, certification, and regulatory requirements.
  • Communicate effectively with residents, families, employees, healthcare providers, and community leadership.
  • Read and complete ISPs, care documentation, assignment sheets, communication reports, schedules, and other required records.
  • Stand and walk for extended periods while completing rounds and supporting resident care.
  • Frequently bend, reach, stoop, kneel, push, pull, and perform movements necessary for resident-care responsibilities.
  • Assist with resident mobility, positioning, and transfers using appropriate techniques, assistance, and equipment.
  • Safely operate floor lifts, sit-to-stand lifts, and other resident-assistance equipment as required.
  • Perform duties involving direct physical contact with residents and potential exposure to bodily fluids and other conditions associated with resident care.
  • Maintain regular and reliable attendance.
  • Work a flexible schedule based on resident and community needs, which may include days, evenings, nights, weekends, holidays, and additional hours.

Responsibilities

  • Coordinate Daily Resident Care: Complete morning and afternoon rounds, review shift updates, speak with care team members, follow up on resident needs, read and understand resident Individualized Service Plans (ISPs), complete scheduled wellness checks, and promptly communicate significant changes or concerns.
  • Lead & Support the Care Team: Provide day-to-day leadership and direction to CNA/Resident Assistants, confirm employee presence and deployment, coordinate and adjust assignments and staffing, train, mentor, coach, and support CNA/Resident Assistants, participate in employee coaching and corrective-action processes, assist with new Resident Assistant orientation and skills checks, monitor employee documentation and assignment completion, ensure employees follow ISPs, safety procedures, and infection-control practices, and provide hands-on resident care when needed.
  • Coordinate Vitals, Weights & Wellness Monitoring: Coordinate and delegate resident weights and vital signs, monitor completion and accuracy, monitor completion of required vital signs, and communicate abnormal findings or concerns.
  • Coordinate Showers, Baths & Laundry: Maintain and coordinate resident shower and bathing schedules, follow up on completion, help coordinate resident laundry responsibilities, and address missed or incomplete services.
  • Support Hospice & Outside Providers: Maintain current information regarding residents receiving hospice services, coordinate hospice schedules, provide appropriate resident information to hospice providers, follow up regarding durable medical equipment (DME) provided through hospice, and communicate with hospice and other outside providers.
  • Support Diet & Nutrition Communication: Help ensure resident ISPs accurately reflect current diet instructions, communicate diet changes promptly, follow up on identified dietary needs and concerns, and communicate resident changes affecting diet or nutrition.
  • Support Life Safety & Quality Assurance: Assist with routine quality and life-safety monitoring, complete or coordinate checks related to elopement risks, DME, resident pendants and call systems, and other assigned quality or safety measures, identify concerns and ensure appropriate follow-up, maintain emergency binders, and follow established resident-safety, infection-control, and emergency procedures.
  • Lead Daily Care Focus: Lead daily Care Focus meetings, coordinate participation, ensure responsibility is communicated to supervisors, gather employee feedback, and escalate urgent concerns.
  • Communicate With Residents, Families & the Care Team: Maintain consistent communication among residents, families, caregivers, CNA/Resident Assistants, LPNs, the DON, and other departments, participate in weekly Clinical Care Coordination meetings, communicate effectively with physicians, pharmacies, hospice providers, and other outside providers, build positive relationships with residents and families, and respond to resident and family questions or concerns.
  • Provide Hands-On Resident Support: Assist residents with activities of daily living, support residents with grooming, bathing, oral hygiene, dressing, eating, toileting, incontinence care, mobility, transfers, and other identified needs, respond promptly to resident pages and alarms, assist residents with appointments and community programs, use proper body mechanics and approved resident-assistance equipment, promote resident independence, comfort, privacy, dignity, choice, and individuality, observe residents for changes in condition or behavior and promptly report concerns, and perform other resident-care responsibilities.
