About The Position

The Wellness Coordinator (LPN) oversees resident care to maintain the highest quality of life for each individual resident. This role requires functioning within the scope of practice according to the State Board of Nursing, administering medications and treatments as prescribed by a physician, and overseeing medication distribution according to policy. The position also involves supervising, evaluating, and potentially suspending personnel according to HCF policy, accurately documenting care provided, and observing, evaluating, and reporting any abnormal findings such as injuries, incidents, skin issues, complaints, suspected abuse/neglect/misappropriation, and unsafe conditions, as well as any significant changes in resident condition. The role includes attending required in-service education, following appropriate Infection Control and Safety Practices, and communicating effectively with residents, staff, and visitors.

Requirements

  • Graduation from an accredited school of nursing
  • Current license as a Licensed Practical Nurse with the State Board of Nursing
  • CPR certification recommended
  • Experience in a position with similar goals and responsibilities
  • Compassionate, team players, social, trustworthy and service-minded
  • Self-motivated, dependable, ethical, and ready to jump in and provide outstanding care and customer service to our residents

  • College or clerical training desirable but not required
