About The Position

The Weekend Wound Care Nurse (RN or LVN/LPN) works at a skilled nursing facility and is responsible for assessing, treating, documenting, and monitoring resident wounds. The nurse collaborates with the interdisciplinary team to support quality care and works weekends, Saturday and Sunday.

The role includes providing wound care in accordance with physician orders and facility protocols, monitoring healing, documenting care in the electronic medical record, communicating wound updates to the care team, and educating staff on wound care and prevention.

Requirements

• Current and valid RN or LVN/LPN license in the state of [State Name].
• Strong clinical assessment and critical thinking skills.
• Ability to work independently and manage time effectively.

Nice To Haves

• At least one year of wound care experience in a long-term care or skilled nursing facility setting is preferred.
• Wound Care Certification (WCC, WOCN, or equivalent) is highly preferred but not required.
• Familiarity with EMR systems such as PointClickCare is a plus.

Responsibilities

• Conduct wound assessments and provide appropriate wound care according to physician orders and facility protocol.
• Monitor wound healing progress and adjust care plans as needed.
• Perform dressing changes, wound debridement when licensed and certified, and pressure injury prevention.
• Document wound assessments, treatments, and progress in the electronic medical record.
• Communicate with physicians, nursing staff, and other care team members about wound status and updates.
• Educate staff on proper wound care techniques and preventative measures.
• Participate in wound care rounds and QAPI meetings as needed.
