About The Position

Potomac Falls Health & Rehabilitation Center is seeking a dedicated and compassionate Wound Care Nurse to join our clinical team. The ideal candidate will be responsible for assessing, treating, and monitoring residents with wounds, skin conditions, and related healthcare needs while collaborating with the interdisciplinary team to promote optimal healing outcomes.

Requirements

• Current RN or LPN license in the Commonwealth of Virginia.
• RN or LPN License Required

Nice To Haves

• Wound Care Certification (WCC, WOCN, or equivalent) preferred, or willingness to obtain certification.
• Previous experience in long-term care, rehabilitation, skilled nursing, or wound care preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal abilities.
• Ability to work collaboratively within a multidisciplinary team.
• Wound certification preferred.

Responsibilities

• Perform comprehensive wound and skin assessments on residents.
• Develop, implement, and evaluate wound care treatment plans.
• Monitor and document wound progress and treatment effectiveness.
• Coordinate care with physicians, nursing staff, rehabilitation therapists, and other healthcare professionals.
• Educate residents, families, and staff on wound prevention and treatment.
• Ensure compliance with all regulatory requirements and facility policies.
• Maintain accurate clinical documentation in the electronic medical record system.
• Participate in quality improvement and infection prevention initiatives.
