About The Position

Eddy Visiting Nurse & Rehab Association has a unique opportunity for an RN looking for a weekend position. This role involves working in SPHP Hospitals to provide home IV education to patients with new infusions, preparing them for discharge and homecare services. The nurse will collaborate with hospital case managers to assess the suitability of homecare and assist with referral processing. IV experience is preferred but not required. This position offers two scheduling options: 12-hour shifts every Saturday & Sunday, qualifying for Full-Time benefits and an elevated Weekend Track rate, or three 8-hour shifts (Friday/Saturday/Sunday or Saturday/Sunday/Monday), qualifying for Part-Time benefits with an experience-based rate. Requirements

  • A current license to practice as a Registered Nurse (RN) in the State of New York
  • 2 years or more of recent experience as a RN
  • Must complete internal IV Therapy course and successful demonstration of skills by line and therapy
  • The RN must have strong evaluation and critical thinking skills
  • The RN must process strong computer and organizational skills
  • The RN must have excellent time management and organizational skills
  • The RN must be able to communicate effectively, verbally and written and possess strong critical thinking skills to problem solve

Nice To Haves

  • IV experience preferred but not required
  • 2 years or more of IV experience
  • Experience in a home care / community health setting
  • Bachelor's degree of nursing

Responsibilities

  • Coordinate smooth patient transitions of care to home health / home infusion pharmacy services from St. Peter’s Hospital and Samaritan Hospital
  • Evaluates referrals for appropriateness for certified home health agency services and home infusion pharmacy services
  • Describes services to patients/families to obtain agreement with services
  • Coordinates with the hospital team, Eddy VNRA, and Empire Home Infusion Service pharmacy to ensure a smooth transition home with home health support
  • Provide education to patient’s and their caregivers for prescribed home infusion therapy
  • Data entry. Upload information into the electronic medical record, and ensuring referral information is complete for the admitting clinician
  • Works closely with Empire Home Infusion Pharmacy to coordination all home-based services (contingent upon patient choice and insurance authorization).
  • Travel will be minimal, but some travel between hospitals will be necessary.
