About The Position

Eddy Visiting Nursing and Rehabilitation Association (EVNRA), a member of St. Peter’s Health Partners, is seeking a Weekend Track Homecare Physical Therapist. This full-time position involves working every Saturday and Sunday with 12-hour day shifts. The role offers independence, the use of cutting-edge technology, competitive compensation, and a strong focus on safety. As a Homecare PT, you will receive a thorough orientation and mentorship, along with strong management support. The position is responsible for admitting patients requiring home-based therapy, primarily post-acute or subacute, and promoting/providing physical therapy services in the home to assist patients in achieving their optimal level of functioning. You will evaluate and treat individuals with functional deficits secondary to neurologic, orthopedic, and medical conditions, caring for one patient at a time. This role is at the forefront of the transition from inpatient settings to community-based care.

Requirements

Graduate of an approved Physical Therapy program with a Bachelor of Science, Master’s degree or Clinical Doctorate of physical therapy degree.
Current NYS PT license and registration
Minimum 1 year of Physical Therapist required.
NYS Clean Driver's License and working vehicle
Care for one patient at a time.
