About The Position
Home Care Eddy Visiting Nurse & Rehab Association is seeking a compassionate and motivated Medical Social Worker to join our dedicated Home Care team. Requirements
- Master's degree in social work is required.
- NYS Social Work Licensure required.
- Excellent communication, assessment, and care coordination skills.
- Valid New York State driver's license and reliable transportation required.
Nice To Haves
- Previous experience in healthcare, home care, hospice, or a related healthcare setting preferred.
Responsibilities
- Conduct comprehensive psychosocial assessments in patients' homes.
- Develop individualized plans of care that address patient and family needs.
- Connect patients and caregivers with community resources, support services, and benefit programs.
- Collaborate closely with nurses, therapists, physicians, and other members of the interdisciplinary care team.
- Participate in case conferences and care planning meetings.
- Advocate for patients and families to promote optimal health, safety, and quality of life.
- Provide emotional support, counseling, and education related to illness, adjustment, and caregiving challenges.
