About The Position
Home Health Care, Inc. is seeking experienced Registered Nurses (RNs) for their weekend admissions team. This role focuses on performing Start of Care visits for new home health patients. The ideal candidate will have a strong understanding of home health admissions, including comprehensive assessments, OASIS documentation, and coordinating patient care. This position offers a unique compensation structure of $200 per Start of Care visit plus mileage reimbursement, with weekend-only scheduling. It's designed for experienced home health nurses who want to leverage their specialized skills for meaningful earning potential without the commitment of ongoing case management.
Requirements
- Active Minnesota RN license.
- Current CPR certification.
- Home health nursing experience required.
- OASIS experience required.
- Strong clinical assessment and documentation abilities.
- Confidence making independent clinical decisions in the field.
- Valid driver's license, active insurance, and reliable transportation.
Nice To Haves
- Start of Care or admission experience strongly preferred.
Responsibilities
- Complete Start of Care admissions for new home health patients.
- Perform comprehensive skilled nursing assessments.
- Complete accurate and timely OASIS documentation.
- Review medications, diagnoses, safety concerns, and disease-management needs.
- Establish individualized plans of care.
- Educate patients and caregivers on their care at home.
- Identify additional nursing, therapy, social work, or other service needs.
- Coordinate with physicians and interdisciplinary clinicians.
- Maintain accurate and compliant electronic documentation.
